Supported Decision Making Services
Supported Decision Making (SDM) is recognized in Minnesota as one of many possible options for a less restrictive alternative to guardianship or conservatorship. SDM is when a person utilizes a team of supporters to help them gather and process informaiton and support them in making their own decisions. Unlike a guardianship or conservatorship, where rights are transferred to the court-appointed representative, in SDM the person retains all their rights. We offer serving as a contracted fiduciary member of a supportive decision-making team as we recognize that sometimes having an independent unbiased professional on the team can provide another level of trusted support.
Care Management & Care Advocate
The current US medical system relies on the patient advocating for their own health and needs at every step of the process. Coordinating between various medical providers of different disciplines and advocating for your health can be a daunting and time-consuming task. We offer private advocacy services to assist clients in obtaining the informaiton they need to make informed consent choices with their medical providers and to ensure their medical providers are receiving the full detailed informaiton they need to best advise their patients.
Healthcare Agent
We serve as Healthcare Agent for clients who want a trusted independent decision-maker to handle their medical decisions if they are unable to communicate or make the decisions themselves. An advance health care directive is a legal document where a person authorizes one or more agents as their Healthcare Agent and often includes instructions and directives to the agent about the person’s wishes. The Healthcare agent has access to medical records and is empowered to make certain decisions, which can include healthcare decisions, end of life decisions, and decisions about handling of someone’s final remains. The appointment of a healthcare agent can be terminated by the client at any time.
